Healthy Homes Block by Block (Healthy Homes) is an innovative means of transforming how health and social needs of the children and families in the Price Hill neighborhood of Cincinnati are met. Since 2013, Healthy Homes has been building a neighborhood-based network of community members who perform door-to-door personal outreach to support the needs and hopes of households with children under the age of six and pregnant women.

Residents of Price Hill experience health, education, and social hardships. While effective programs exist to serve Price Hill residents, the most at-risk families remain isolated from service pathways. To connect families who do not walk through clinic and agency doors, Healthy Homes engages them on their own doorsteps.
